ScienceDaily (Sep. 2, 2011) — Researchers at The Pennsylvania State University College of Medicine, Hershey, Pennsylvania have discovered the mechanism by which a low dose of the opioid antagonist naltrexone (LDN), an agent used clinically (off-label) to treat cancer and autoimmune diseases, exerts a profound inhibitory effect on cell proliferation.


It has been postulated that opioid receptor blockade by LDN provokes a compensatory elevation in endogenous opioids and opioid receptors that can function after LDN is no longer available. Using a novel tissue culture model of LDN action, the mechanism of LDN has been found to target the opioid growth factor (OGF, [Met5]-enkephalin) and OGF receptor (OGFr) axis. This discovery, reported in the September 2011 issue of Experimental Biology and Medicine, provides new insights into the molecular pathway utilized by an increasingly important clinically prescribed agent that serves as a basic biological regulator of cell proliferative events related to pathobiological states such as cancer and autoimmune diseases

Although the antitumor effects of opioid antagonists were first noted by Drs. Zagon and McLaughlin in 1981 (Life Sci. 28:1095-1102, 1981), the first full reports about opioid antagonists modulating growth processes occurred in 1983 (Science 221:671-673; ibid, 221:1179-1180). This led to the hypothesis that endogenous opioid systems play a role in cancer, development, and cellular renewal (Life Sci. 35:409-416, 1984; ibid, 35:2057-2064, 1984). These papers revealed that a short-term opioid receptor blockade with naltrexone (NTX), a general opioid receptor antagonist devoid of intrinsic activity, results in an elevation in endogenous opioids and opioid receptors in response to the opioid receptor blockade. Interference of opioid peptide-opioid receptor interactions for a short time each day (4-6 hr) with LDN provided a subsequent window of time (18-20 hr) for the increased levels of endogenous opioids and opioid receptors to interface and elicit a robust functional response: inhibition of cell proliferation. The question that now can be addressed is which endogenous opioid(s) and opioid receptor(s) are responsible for LDN's effects on cell proliferative processes.

The present study was structured to focus on the relationship of endogenous opioid pathways and the repercussions of intermittent opioid receptor blockade with regard to cell proliferation. A unique tissue culture model of LDN using a short-term exposure to NTX was developed, thereby avoiding the confounding variables introduced by systemic influences and allowing a dissection of the biological events involved. Screening of a wide variety of opioids (some selective for specific opioid receptors) revealed that only exogenous OGF had a profound effect on depressing cell proliferation. Removal of endogenous OGF by antibody neutralization in cultures given a short-term opioid receptor blockade by NTX eliminated the repressive effects of this peptide on cell proliferation, indicating that the repercussions of short-term NTX exposure in vitro was dependent on OGF. Short-term NTX blockade continued to exert a negative effect on cell proliferation even when the classical opioid receptors, μ, δ, and κ, were knocked down by siRNA technology. However, short-term NTX treatment did not repress cell proliferation when cells were subjected to siRNA to the non-classical opioid receptor, OGFr. These results indicate that the effects of short-term NTX in vitro are dependent on the OGF-OGFr axis. Previous studies have shown that the OGF-OGFr axis regulates cell proliferation by altering the G1/S phase of the cell cycle through the cyclin-dependent inhibitory kinases p16 and p21. Knockdown experiments with siRNA show that a short interval of exposure to NTX in tissue culture required p16 and/or p21 in order to have a functional outcome on cellular processes.

The research team was composed of Dr. Ian S. Zagon, Distinguished University Professor, and Dr. Patricia J. McLaughlin, Professor, along with Dr. Renee N. Donahue, in the Department of Neural & Behavioral Sciences. Drs. Zagon and McLaughlin not only discovered the phenomenon of LDN, and subsequently the OGF-OGFr axis, but have been at the forefront of translating their findings of LDN -- and OGF -- from the bench to the bedside. LDN has proven successful in Phase I and II clinical trials in the treatment of Crohn's disease, and OGF has been reported to be safe and efficacious for the treatment of advanced pancreatic cancer.

Co-author Dr. McLaughlin states: "Now that we know LDN uses the OGF-OGFr axis as the pathway to control the cell cycle, this expands our arsenal of biological-based treatment modalities to bring about a change in disease states reliant on cell proliferation that not only includes LDN, but exogenous OGF and the imidazoquinoline, imiquimod. This information also provides the basis for a rational approach to the design of diagnostic tools and measures of therapeutic efficacy."

Dr. Donahue, who has devoted a concentrated effort on improving the health of women through research explains: "This study joins a series of other investigations (Amer. J. Physiol. 296:R1716-1725, 2009; ibid, 297:R1154-R1161, 2009; Gynecol. Oncol. 122:382-388, 2011; J. Cancer Therapy 2:110-124, 2011; Exp. Biol. Med., in press, 2011) demonstrating that both LDN and OGF offer powerful treatments to combat a devastating cancer that strikes over 20,000 women in the U.S. each year and stands as the 5th leading cause of cancer-related deaths in females. Thus, one should not lose sight that the OGF-OGFr axis is a new frontier in understanding the pathogenesis and treatment of a cancer that has been, up to now, recalcitrant to conventional therapies."

Dr. Zagon adds: "The exciting results that the mechanism of LDN uses the OGF-OGFr axis, brings together two very important opioid-based treatment modalities under one umbrella. This common denominator in a physiological pathway does much in now directing our attention to how the OGF-OGFr system works, and explains why an opioid agonist (i.e., OGF) and antagonist (NTX in the form of LDN) have the same effects. Moreover, LDN is an oral medication, generic, inexpensive, and non-toxic, and has been documented to alter the course of both neoplasias and autoimmune diseases such as Crohn's and multiple sclerosis, making this drug especially attractive as a therapeutic agent. The fact that OGF has been found to be a potent anti-inflammatory agent (Immunobiology 216:173-183, 2011; ibid, 216:579-590, 2011) also opens the door to the potential treatment of diseases of the immune system (HIV/ADS) infections, hypersensitivity, and neurodegeneration, which involve cell proliferation, thereby widening the benefit from therapeutic manipulation of the OGF-OGFr axis by LDN."

Dr. Steven R. Goodman, Editor-in-Chief of Experimental Biology and Medicine said: "These researchers from the Milton S. Hershey Medical Center have made the important discovery of the mechanism by which a low dose of the opioid antagonist naltrexone (LDN) can suppress cell proliferative-related disorders such as cancer and autoimmune diseases. This is an exciting new direction for future therapy."

Veel klassieke aandoeningen worden gekenmerkt door een te laag aantal b-endorfinen en dat is net wat LDN terug zal aanmaken. Je immuunsysteem komt terug in balans.

Als je weerstand slecht is, wordt dit gekenmerkt door een te laag aantal b-endorfinen. Je kan het dus zelf verbeteren door LDN te nemen en het lichaam zijn eigen functies weer te laten uitvoeren.
Lage dosis Naltrexone (LDN) wordt nu vooral gebruikt als regulator van het immuunsysteem, om hulp te bieden aan patiënten met auto-immuunziekten, en met centrale zenuwstelselaandoeningen.

MS, fibromyalgie, lupus, kanker, ... de resultaten zijn aanmoedigend voor de vele patienten én artsen.

Hoewel het FDA dit medicijn oorsponkelijk niet voorschrijft voor deze aandoeningen, zijn artsen toegestaan om LDN 'off-label' voorschrijven voor welk gebruik dat ze denken dat het passend is.

De ogenschijnlijk uiteenlopende aandoeningen die therapeutisch effect blijken te hebben van LDN, hebben allemaal een gemeenschappelijke factor : een verhoogd niveau van endorfinen (beta en metenkephalin endorfinen), het verdubbelt en verdrievoudigt soms het aantal en de activiteiten van natural killers cells, verhoogt cytotoxic CD8+ T cellen in circulatie, het induceert apoptosis door een verhoogd aantal van endorfine tumorcel receptoren.

Immunoexcitotoxicity as a central mechanism in chronic traumatic encephalopathy

De oorzaak van deze neurologische verstoring ligt o.a. in de endorfine receptoren. Die vervullen een heleboel functies.

Deze receptoren zijn afhankelijk van wat je eet en welke enzymen dit voedsel afbreken. Als er daar iets misloopt, zit je in de problemen. Net zo'n specifiek enzym (DPP IV) is verbonden met neuropeptide Y dat nu geciteerd is in een onderzoek als cruciaal in chronische pijn (bv fibromyalgie).

Hetzelfde enzym kwam ook ter sprake op de NIH workshop van 7 en 8 april jl. tijdens de samenvatting van Dr Nancy Klimas over het werk van Broderick. Ditmaal met de duidelijke boodschap dat het een sleutelrol speelt in immuniteit, meer bepaald in het signaliseringsproces van lymfocyten.



Het voedsel waarvan sprake zijn natuurlijke opiaten, oftewel gluten (= brood, pasta, koekjes e.d.), caseine (= melkprodukten), soya en spinazie. Veel chronisch zieke mensen blijken een slecht werkend DPP IV enzym te hebben waardoor deze stoffen niet goed afgebroken worden. Deze bereiken giftige concentraties in het bloed en breken de endorfine-receptoren af. Je immuunsysteem is om zeep, je brein kan niet meer nadenken, je zenuwstelsel staal tilt, je emotioneel welzijn gaat eraan.
Zeer recent is dus ook gebleken dat het een rechtstreeks verband houdt met chronische pijn.

Belangrijk is ook te begrijpen dat pijnstillers van de opiaten-familie hetzelfde vernietigende effect hebben op je endorfine-receptoren. Probeer daarom over te schakelen op een andere soort pijnstiller als je je immuunsysteem terug aan de praat wil krijgen.

LDN stimuleert de aanmaak van nieuwe endorfine-receptoren in mijn brein, dus mijn hersenen kunnen weer werken.

En mijn brein is de computer van mijn lichaam ... Het weet beter dan welke specialist ook wat er moet gerepareerd worden.

Een chronisch ziek lichaam genezen is zoals een oud huis repareren. Veel kosten en oude rommel eruit. Zwoegen, zweten en afzien.
Maar beetje bij beetje wordt het mooier en beter.

Naltrexone wordt in België enkel verkocht als Nalorex 50mg. Het moet thuis of door de apotheker dus nog verdund worden tot je een halve milligram tot maximaal 3,5 - 4 milligram kan innemen = Low Dose Naltrexone.

In Nederland wordt het ook op voorschrift aangemaakt. Tabletten van 0,5 mg zijn technisch gezien mogelijk. Zoniet kan je het verkrijgen onder de merknaam Revia. Dat is de 50mg dosering. Die moet je wel zelf verdunnen.

Benodigdheden:
1 tablet 50mg Nalorex of Revia
1 ampul 10ml fysiologisch zout (NaCL 0,9%)
1 druppelpipetje 10 ml met donker flesje

Bereiding:
Doe een tablet van 50mg in het flesje en voeg de 10ml ampul fysiologisch zout toe.
Het duurt ongeveer twee uur vooraleer de ampul opgelost is.
Je krijgt dan een concentratie van 0,5mg LDN per 2 druppels.
Het flesje bevat in totaal 200 druppels = 50mg LDN
Bewaar maximum 1 maand in de koelkast

Inname:
Schud het flesje
1 druppel = 0,25 mg
2 druppels = 0,5 mg

De LDN in een bekertje water druppelen en net voor slapengaan innemen.


Je eigen apotheker zou het ook moeten kunnen leveren onder tabletvorm van 0,5 mg. Het zijn tenslotte pillendraaiers nietwaar.


ME een toxidroom?
Men spreekt hiervan als patienten op de spoed worden opgenomen bij een overdosis medicatie (pijnstillers, antidepressiva, betablokkers, ...), vergif, organofosfaten, drugs, ...


Interessant om weten is dat de standaardbehandeling die men geeft bij een vergiftiging in een ziekenhuis Naloxone is, oftewel de injecteerbare versie van Naltrexone, zelfs bij kleine kinderen.

Verder gebruikt men vaak actieve kool voor de darmen, darmlavage en bicarbonaat voor het alkaliniseren van de urine. Men vermeldt ook lysomusil (NAC), metyleen blauw, ...

Degenen die mijn blog al een tijdje volgen, zullen ongetwijfeld een reeks handelingen herkennen die mijn gezondheid hebben doen vooruitgaan.

Namelijk:

- low dose naltrexone

- leverreiniging (wat ook de darmen reinigt)
- NAC
- ...

Afgezien van acute vergiftiging zijn er verschillende redenen waarom mensen slecht bovenvermelde toxische stoffen kunnen afbreken:

- je bent glutenintolerant
- je hebt een genetische afwijking (cytochroom P450)
-je lever is chronisch overbelast wegens ziekte
- ...

Low Dose Naltrexone kan je dus helpen bij het stelselmatig ontgiften.

Wat is een zeer veilige opstartdosis?
Week 1 - 4: 0,25 mg

Voeg volgende kolommen toe:

Kleur systematisch de blokjes van de uren dat je slaapt ROOD, ligt GEEL, zit of actief bent GROEN.

Na enkele weken tijd zal je minder rood en geel op je kalender zien verschijnen. In de plaats komt meer groen.

Merk op dat er meer en meer groene zones zijn naarmate de tijd vordert.

Ze worden voorlopig nog wel steevast gevolgd door een duidelijke rode zone (post exertional malaise) of extra gele zone maar toch, verrassend meer groen!


Zelf verhoog ik de dosis enkel op zondagavond wegens potentiële verhoging van temperatuur en extra ziektegevoel dat dit vaak zich meebrengt. Dat kan ik uitzieken tijdens de week om dan weer uit bed te kunnen tegen het volgende weekend als de kinderen thuis zijn.

Waarom dat ziektegevoel? LDN maakt extra endorfinen aan en bij elke dosisverhoging, zijn er dus meer manschappen om indringers te lijf te gaan. Na een week hebben ze het grootste werk geleverd.


LDN is geen stand alone behandeling. 30% van de bevolking verdraagt geen gluten, melk, soya en spinazie. Echter gluten en co gooit roet in het eten van de LDN omdat het een opiaten antagonist is. Het belet dat opiaten zich gaan hechten aan de mu opioide receptoren van de endorfinen.


Het zal ervoor zorgen dat het minder goed aanslaat en dat je meer ziekte-gevoel krijgt. Probeer gluten en melk te vermijden. Het is zowieso een bron van ellende maar dit is niet de plek om er over uit te wijden.
Overweeg serieus om over te schakelen op glutenvrij eten. Mensen beseffen pas als ze het 6 maanden niet meer eten, hoe bevrijdend dat kan zijn voor de geest en lichaam.

Welke pijnstillers kan je NIET nemen met LDN?

Alles van de opiaten-familie.
Waarom niet? LDN blokkeert tijdelijk de opioide receptoren waardoor deze medicijnen geen effect hebben. Neem dus enkel LDN als deze medicatie volledig uit je lichaam is. Zoniet riskeer je ernstige pijn.

Mensen die langdurig pijnstillers van de opiaten familie nemen, kunnen best gedurende een tweetal weken langzaam overschakelen op een ander medicijn vooraleer ze aan LDN beginnen.

Welke pijnstillers zijn WEL toegelaten met LDN?

Moxxor, aspirin, Tylenol®, Advil®, Motrin®, Aleve®, Naprosyn®, Ansaid®, Dolobid®, Orudis®, Voltaren®, Feldene®, Mobic® en het voedingssupplement D-Phenylalanine (zie hieronder).


Ben je net gestart met LDN en heb je last van de nevenwerking pijn?
Of erger, ben je een chronische pijnpatient?

Wist je dat eigen endorfinen sterker zijn dan morfine?

1) Lig je te kronkelen in je bed en is de pijn niet te harden? Geen paniek. Probeer als eerste oplossing de dosis te verlagen. Misschien ben je net als ik gebaat met een lagere instapdosis: 0,25 mg gedurende de eerste 4 weken.

2) ik neem D-phenylalaline 500 mg. Dat is een natuurlijk middel dat de pijn fel zal minderen. Neem eentje 's morgens en eentje in de namiddag. Na enkele weken zal de pijn zakken. Je lichaam heeft inmiddels endorfinen aangemaakt dankzij de LDN die de pijn zal minderen. Dat is het moment om de dosis LDN te verhogen naar 0,5 mg. Het werkt ook goed tegen chemische overgevoeligheid trouwens.

Wat is D-phenylalaline?

Fenylalanine kan niet door het lichaam worden aangemaakt, het is daarom een essentieel aminozuur.
Het is een voedingssupplement en pijnstiller.
L-fenylalanine maakt deel uit van een aantal psychoactieve medicijnen, evenals belangrijke lichaamsstoffen, zoals acetylcholine, vasopressine, cholecystokininen, enkefalinen en endorfinen.

Endorfinen en enkefalinen zijn substanties die het lichaam aanmaakt als we zware lichamelijke inspanning (sport) verrichten of wanneer we positieve emoties ondervinden.

Deze door het eigen lichaam aangemaakte morfineachtige stoffen (endogene opiaten) maken het lichaam minder gevoelig voor (of bewust van) pijn.

Bovendien hebben deze stoffen een stemmingsverbeterend effect. Ze worden in verband gebracht met een positievere levensinstelling, een verhoogde alertheid, verbetering van de geheugenfunctie, een verhoogde vitaliteit en vergroting van de seksuele interesse. 

Deze door het eigen lichaam aangemaakte morfineachtige stoffen (endogene opiaten) maken het lichaam minder gevoelig voor (of bewust van) pijn. Hiervoor doen we het uiteraard!

Oppassen, niet nemen als je zwanger bent, de ziekte PKU of aan hoge bloeddruk lijdt. Lees steeds waarschuwingen bij supplementen. Mensen met een huidkanker (nu of vroeger) mogen dit niet nemen.
Zoals bij alle supplementen, zoek steeds naar extra informatie. Do your homework. Het is jouw lichaam.

5) Zelf ben ik sedert enkele jaren getrouwe gebruiker van Topamax 100 mg. Na 30 jaar dagelijkse hoofdpijn die zowat ondragelijke proporties had aangenomen, bleek dit een oplossing.
