Abstract

BACKGROUND:

Chronic Fatigue Syndrome/Myalgic Encephalomyelitis (CFS/ME) is characterised by severe prolonged fatigue, and decreases in cognition and other physiological functions, resulting in severe loss of quality of life, difficult clinical management and high costs to the health care system. To date there is no proven pathomechanism to satisfactorily explain this disorder. Studies have identified abnormalities in immune function but these data are inconsistent. We investigated the profile of markers of immune function (including novel markers) in CFS/ME patients.

METHODS:

We included 95 CFS/ME patients and 50 healthy controls. All participants were assessed on natural killer (NK) and CD8(+) T cell cytotoxic activities, Th1 and Th2 cytokine profile of CD4(+) T cells, expression of vasoactive intestinal peptide receptor 2 (VPACR2), levels of NK phenotypes (CD56(bright) and CD56(dim)) and regulatory T cells expressing FoxP3 transcription factor.

RESULTS:

Compared to healthy individuals, CFS/ME patients displayed significant increases in IL-10, IFN-γ, TNF-α, CD4(+)CD25(+) T cells, FoxP3 and VPACR2 expression. Cytotoxic activity of NK and CD8(+) T cells and NK phenotypes, in particular the CD56(bright) NK cells were significantly decreased in CFS/ME patients. Additionally granzyme A and granzyme K expression were reduced while expression levels of perforin were significantly increased in the CFS/ME population relative to the control population. These data suggest significant dysregulation of the immune system in CFS/ME patients.

CONCLUSIONS:

Our study found immunological abnormalities which may serve as biomarkers in CFS/ME patients with potential for an application as a diagnostic tool.

Abstract

Chronic fatigue syndrome is a disorder characterised by prolonged fatigue and debility and is mostly associated with post-infection sequelae although ongoing infection is unproven. Immunological aberration is likely and this may prove to be associated with an expanding group of vasoactive neuropeptides in the context of molecular mimicry and inappropriate immunological memory.

Vasoactive neuropeptides including vasoactive intestinal peptide (VIP) and pituitary adenylate activating polypeptide (PACAP) belong to the secretin/glucagon superfamily and act as hormones, neurotransmitters, immune modulators and neurotrophes. They are readily catalysed to smaller peptide fragments by antibody hydrolysis. They and their binding sites are immunogenic and are known to be associated with a range of autoimmune conditions.

Vasoactive neuropeptides are widely distributed in the body particularly in the central, autonomic and peripheral nervous systems and have been identified in the gut, adrenal gland, reproductive organs, vasculature, blood cells and other tissues. They have a vital role in maintaining vascular flow in organs, and in thermoregulation, memory and concentration. They are co-transmitters for acetylcholine, nitric oxide, endogenous opioids and insulin, are potent immune regulators with primarily anti-inflammatory activity, and have a significant role in protection of the nervous system to toxic assault, promotion of neural development and the maintenance of homeostasis.

This paper describes a biologically plausible mechanism for the development of CFS based on loss of immunological tolerance to the vasoactive neuropeptides following infection, significant physical exercise or de novo. It is proposed that release of these substances is accompanied by a loss of tolerance either to them or their receptor binding sites in CFS. Such an occurrence would have predictably serious consequences resulting from compromised function of the key roles these substances perform.

All documented symptoms of CFS are explained by vasoactive neuropeptide compromise, namely fatigue and nervous system dysfunction through impaired acetylcholine activity, myalgia through nitric oxide and endogenous opioid dysfunction, chemical sensitivity through peroxynitrite and adenosine dysfunction, and immunological disturbance through changes in immune modulation. Perverse immunological memory established against these substances or their receptors may be the reason for the protracted nature of this condition.

The novel status of these substances together with their extremely small concentrations in blood and tissues means that clinical research into them is still in its infancy. A biologically plausible theory of CFS causation associated with vasoactive neuropeptide dysfunction would promote a coherent and systematic approach to research into this and other possibly associated disabling conditions.

In het Tijdschrift voor Cliëntgerichte Psychotherapie reageert Rob Arnoldus op een artikel van Chaja Kaufmann (Onverklaard maakt onbemind. Het werken met onverklaarde lichamelijke klachten in de groep. Tijdschrift Cliëntgerichte Psychotherapie. 2011. 49(1):38-53), waarin ze stelt dat o.a. ME/CVS het gevolg is van een verstoorde ontwikkeling van het lichaamsbesef.

Met belangstelling heb ik kennisgenomen van een recent artikel van Kaufmann over cliënten met ‘lichamelijk onverklaarde klachten’. Bij dezen maak ik gebruik van de mogelijkheid een aantal fundamentele kritische kanttekeningen te plaatsen bij enerzijds de gebruikte terminologie en anderzijds de aangedragen primair psychologische verklaringen van de klachten. De verklaringen (hechtingstheorieën) worden gepresenteerd als feiten terwijl het hypothetische uitspraken betreft. Ter illustratie zal ik kort stilstaan bij de labels ‘CVS’ en ‘chronische vermoeidheid’.



Vooreerst stel ik dat de term ‘psychosomatische klacht/ziekte’ niet geschikt is als aanduiding van lichamelijk onverklaarde klachten. De term verwijst immers naar symptomen/syndromen die voornamelijk door psychosociale factoren kunnen worden verklaard. Ook het gebruik van de aanduiding ‘onverklaarde lichamelijke klachten’ leidt tot verwarring omdat de genoemde voorbeelden niet op voorhand lichamelijk onverklaarbaar zijn.



Het OLK-concept ontbeert de nodige begripsvaliditeit. Medisch psychologe Bramsen (2010a) constateerde in GZ-Psychologie dan ook terecht dat onderzoek dat gebaseerd is op de idee dat (1) medisch onverklaard hetzelfde is als medisch onverklaarbaar, (2) en dat er een syndroom bestaat, het MOK-syndroom, dat precies kan worden afgebakend langs de grenzen van de huidige medische kennis, en dat (3) resultaten uit biomedisch onderzoek negeert, in strijd is met de principes van zuiver wetenschappelijk onderzoek. Bramsen (2010b) waarschuwt in een recent verschenen boekhoofdstuk over Evidence(?)-based practice voor de valkuilen en mythes over somatisatie en zogenaamde medisch onverklaarde klachten. Zij geeft tevens voorbeelden van deugdelijk biomedisch onderzoek met aanwijzingen voor een biologische verklaring van de vermeende psychosomatische syndromen waaronder we in een recent verleden ook epilepsie, MS en Gilles de la Tourette konden scharen.



Kaufmann beschouwt OLK als een dergelijk MOK-syndroom en veronderstelt op voorhand dat er bij al deze mensen sprake is van een disbalans tussen het sympatische en parasympatische zenuwstelsel (zie p. 41). Met een dergelijke niet te toetsen generalisatie slaat Kaufmann de plank volledig mis. Zo zijn de ‘klachten’, van ME- en/ of CVS-patiënten, mits de diagnose gesteld wordt aan de hand van deugdelijke criteria, wel degelijk biologisch verklaarbaar (Twisk, Arnoldus & Maes, 2011) en in de meeste gevallen niet terug te voeren op die (vermeende) disbalans. De internationaal gangbare Fukuda-criteria (Fukuda, Straus, Hickie, Sharpe, Dobbins & Komaroff, 1994) sluiten bij CVS psychiatrische aandoeningen uit. Hechtingsproblematiek als verklaring voor de bij deze groep patiënten veelvoorkomende objectiveerbare post-exertional malaise en immunologische problematiek ligt niet voor de hand. Het verdient vermelding dat Courjaret, Schotte, Wijnants, Moorkens en Cosyns (2009) geen DSM-IV-axis-II persoonlijkheidsstoornissen aantroffen bij CVS-patiënten en dat Taylor en Jason (2001) hebben aangetoond dat CVS-patiënten (Fukuda-criteria) niet meer dan gezonden kampen met trauma’s als gevolg van mishandeling of seksueel misbruik. Ook sec ‘de klacht’ chronische moeheid impliceert op voorhand geen hechtingsproblematiek, met of zonder alexithyme. Als de huisarts het niet of nog niet weet dan betekent dat niet dat de psycholoog de cliënt op basis van een vermoeden van psychosomatiek met een gerust hart kan gaan behandelen.



Zo trof Newton (Newton, Mabillard, Scott, Hoad & Spickett, 2010) in haar kliniek voor patiënten met ogenschijnlijk ‘onverklaarde moeheid’ vooral patiënten met diverse aantoonbare somatische klachten aan, waaronder de veelal niet onderkende orthostatische intolerantie (problemen met bloeddoorstroming) waarmee een subgroep van patiënten met chronische moeheid kampt. Menig arts – en laat staan psycholoog – is (nog) niet op de hoogte van de meest recente wetenschappelijke literatuur inzake de veronderstelde ‘OLK-aandoeningen’ en/of is niet geschoold om met controversiële ziektebeelden om te gaan. Recent onderzoek van Norheim, Jonsonn en Omdal (2011) wijzen op een reeks plausibele, complexe biologische verklaringen, waaronder oxidatieve stress en mitochondriale afwijkingen voor de vaak ‘lichamelijk (nog) onverklaarde’ chronische vermoeidheidsklachten.



Kaufmann noemt de OLK-cliënten hardnekkiger, afhankelijker en meer timide (zie p.42). Patiënten met vergaarbakdiagnoses waaronder we ook OLK kunnen scharen, krijgen voortdurend te maken met een overdaad aan onbewezen speculatieve theorieën over hun persoonlijkheid of gedrag. Zo zouden de veelal vrouwelijke patiënten met het CVS-label kampen met perfectionisme, overcompensatie van kinderlijk gemis, te jachtig gedrag, risicomijding etc. etc. De OLK-werk- en denkwijze draagt bij aan de onterechte ‘psychologisering’ van controversiële ziektebeelden en bouwt voort op het in methodologisch opzicht regelmatig ondeugdelijk onderzoek naar MOK of somatisatie (vgl. Bramsen, 2010b). Dit draagt er toe bij dat grote groepen patiënten geconfronteerd worden met uitsluiting van sociale voorzieningen, sociaal isolement en ongeloof. De Schotse psychologe Dickson spreekt in die context van de delegitimation experience (Dickson, 2007).



De mijns inziens therapeutgestuurde benadering van Kaufmann staat haaks op de daadwerkelijke cliëntgestuurde benadering die bijvoorbeeld de Australische psychologen Thomas en Bosch (2005) voorstaan bij patiënten met het CVS-label. Thomas en Bosch nemen nadrukkelijk afstand van de psychosomatische duiding van CVS en pleiten voor een benadering op basis van respect met aandacht voor het sociaal isolement waarin de patiënten verkeren en voor de constante vernedering die deze patiënten moeten ondergaan.



Bij de keuze voor een psychologische behandeling spelen ethische overwegingen een belangrijke rol. Zo vereist het NIP dat de cliënt wordt geïnformeerd over alternatieve verklaringen van de klachten. Tijdens een recente presentatie over ‘onzichtbare ziektebeelden’ (waaronder chronische vermoeidheid en fibromyalgie) maakte de Canadese ethica Shannon (2010) het publiek attent op de schadelijke gevolgen van het mind over matter-denken. Het lot van de overleden Tiffany Pfaff, een patiënte met Creutzfeldt-Jakob, stemt in die context tot nadenken. Zij en haar familie zagen de door behandelaars benoemde conversiestoornis als een adequate en onbetwistbare definitie van de situatie (Bramsen, 2010b). Met de verspreiding van het generaliserende OLK-denken en het propageren van het op conversiestoornissen afgestemde ‘verklaringsmodel’ van Moene en Rumke (zie p. 46) neemt de kans op dergelijke medische missers en iatrogene schade toe. Er is dan ook voldoende aanleiding om afstand te nemen van de vooringenomen opstelling van Kaufmann en om de inhoud van de voorgestelde ‘psycho-educatie’ (zie p. 43), in die context ter discussie te stellen.



Bij wijze van afsluiting een dringend pleidooi om bijzonder voorzichtig te zijn met psychosomatische duidingen, zeker in het geval van weinig afgebakende en onzorgvuldig gediagnosticeerde patiëntenpopulaties. De verwijzing naar DSM-IV-TR, 2005 op de eerste pagina (zie p. 38) in het artikel van Kaufmann voldoet geenszins om het vermeende psychosomatische karakter en de daarop afgestemde veelal ‘geruststellende’ behandeling van de genoemde klachten te legitimeren. Deze door Shannon geciteerde uitspraak van John Stuart Mill kan bij de bejegening van (deze) zieke mensen als inspiratiebron dienen:

‘I’d rather be a human dissatisfied than a pig satisfied; I’d rather be Socrates dissatisfied than a fool satisfied (…).’ In het geval van controversiële en/of ‘onzichtbare’ ziektebeelden is een niet vooringenomen cliëntgestuurde benadering de enig aangewezen weg.

Vitamin D crucial to activating immune defenses

Scientists from the Department of International Health, Immunology and Microbiology have discovered that Vitamin D is crucial to activating our immune defenses and that without sufficient intake of the vitamin, the killer cells of the immune system - T cells - will not be able to react to and fight off serious, life-threatening infections in the body.

For T cells to detect and kill foreign pathogens such as clumps of bacteria or deadly viruses, the cells must first be ‘triggered' into action and ‘transform' from inactive and harmless immune cells into killer cells that are primed to seek out and destroy all traces of a foreign pathogen.

The researchers found that the T cells rely on vitamin D in order to activate and they would remain dormant, ‘naïve' to the possibility of threat if vitamin D is lacking in the blood.

- "We have discovered that the first stage in the activation of a T cell involves vitamin D, explains Professor Carsten Geisler from the Department of International Health, Immunology and Microbiology. When a T cell is exposed to a foreign pathogen, it has an immediate biochemical reaction and extends a signaling device or ‘antenna' known as a vitamin D receptor, with which it search for vitamin D. This means that the T cell must have vitamin D or activation of the cell will cease. If the T cells cannot find enough vitamin D in the blood, they won't even begin to mobilise."

T cells that are successfully activated transform into one of two types of immune cell. They either become killer cells that will attack and destroy all cells carrying traces of a foreign pathogen or they become helper cells that assist the immune system in acquiring "memory". The helper cells send messages to the immune system, passing on knowledge about the pathogen so that the immune system can recognize and remember it at their next encounter and launch a more efficient and enhanced immune response. T cells form part of the adaptive immune system, which means that they function by teaching the immune system to recognize and adapt to constantly changing threats.

Activating and Deactivating the Immune System

For the research team, identifying the role of vitamin D in the activation of T cells has been a major breakthrough.

- "Scientists have known for a long time that vitamin D is important for calcium absorption and the vitamin has also been implicated in diseases such as cancer and multiple sclerosis, but what we didn't realize is how crucial vitamin D is for actually activating the immune system - which we know now."

The discovery, the scientists believe, provides much needed information about the immune system and will help them regulate the immune response. This is important not only in fighting disease but also in dealing with anti-immune reactions of the body and the rejection of transplanted organs. Active T cells multiply at an explosive rate and can create an inflammatory environment with serious consequences for the body. After organ transplants, e.g. T cells can attack the donor organ as a "foreign invader". In autoimmune disease, hypersensitive T cells mistake fragments of the body's own cells for foreign pathogens, leading to the body launching an attack upon itself.

The research team were also able to track the biochemical sequence of the transformation of an inactive T cell to an active cell, and thus they could intervene at several points to modulate the immune response. Inactive or ‘naïve' T cells crucially contain neither the vitamin D receptor nor a specific molecule (PLCgamma1) that would enable the cell to deliver an antigen specific response.

The findings continues Professor Geisler "could help us to combat infectious diseases and global epidemics. They will be of particular use when developing new vaccines, which work precisely on the basis of both training our immune systems to react and suppressing the body's natural defenses in situations where this is important - as is the case with organ transplants and autoimmune disease."

Most Vitamin D is produced as a natural byproduct of the skin's exposure to sunlight. It can also be found in fish liver oil, eggs and fatty fish such as salmon, herring and mackerel or can be taken as a dietary supplement.

The results of another recent study, this one published in the March 2010 issue of Journal of Clinical Endocrinology and Metabolism, found that :

- 59 percent of the world's population lacks clinically appropriate amounts of vitamin D in their bodies. 25 %  of those were found to have seriously low levels!

For optimum health, vitamin D blood levels should be around 50-80 ng/mL (125-200 nmol/L).

Dag 498 Energy!

Die spuitjes neuraal therapie beginnen weer te werken. In de voormiddag ging het nog wat traag en ben ik blijven liggen maar tegen de middag had ik al een maaltijd opgewarmd voor de kinderen.


De auto riep om een wasbeurt en zonder al te veel moeite heb ik twee emmers tot buiten gedragen. De oudste zoon bracht de stofzuiger mee, daar had ik geen zin meer in. Samen was de klus snel geklaard. Niet moeilijk gezien de (kleine) omvang van dat autootje.

De vermoeidheid was even te voelen tijdens de wasbeurt maar ebde even snel weg na een korte rustperiode. De kinderen laadden vervolgens een paar verroeste fietsen in de auto die ik zelf naar het containerpark om de hoek reed. Een vriendelijke man heeft ze eruit gehaald en weggezet.

Eenmaal thuis ben ik de moestuin ingegaan. Gewapend met een onkruidwieder ben ik op zoek gegaan naar de groenten verscholen tussen de meterhoge distels, brandnetels en weidebloemen. Vanavond staat er op het menu: boterboontjes, verse sla en paksoi.

Dat noemen we nog eens een rijkgevulde dag!

Afspraak bij de neuraal arts. Het spreekuur is ontiegelijk vroeg en de wekker is ongenadig. De vakantie heeft me volgens de arts duidelijk deugd gedaan en ook qua gewicht ben ik wat bijgekomen.


De spuitjes zijn identiek als de vorige keer maar ze doen minder pijn behalve in het punt in de borstkast. Ze neemt nauwkeurig nota van de symptomen tijdens de vakantie.

Geen gekke plannen vandaag afgezien van een paar wasmachines die ik uiteindelijk wijselijk laat staan. Vandaag wordt het lezen en een verloren rit naar de gesloten bibliotheek.

Dag 495 Korte nacht

De nacht was extreem kort en de benjamin zoon wekt me vrolijk om samen halfeen ‘s middags. “Opstaan moeder, het ontbijt staat klaar.” Het feit dat ik net aan het dromen deert hem niet, integendeel, stel dat het slechte droom was, heeft hij me net gered!


Amper een uur later lig ik weer in dromenland, de verloren uren in te halen. Pas tegen valavond ben ik weer wat mens.
