Kayser-Fleischer Ring or mononucleosis?

Kayser-Fleischer Ring


The ring encircling the iris of the eye is known as ‘Kayser-Fleischer ring’. These are dark rings which are the result of copper deposition. This copper deposition occurs in patients suffering from liver related diseases. Found in descemet’s membrane and can extend into tabecular meshwork. It first appears as a crescent at top of the cornea.

This ring for identification requires a slit-lamp examination in its early stage. These rings are considered as a sign of Wilson’s disease and may be a result of abnormalities in basal ganglia of the brain, liver cirrhosis, muscle rigidity and involuntary movements. Copper chelation can be used as a therapy.

When hepatitis develops, patients are often thought to have infectious hepatitis or infectious mononucleosis when they actually have Wilson disease hepatitis.

Eat less opioids, have less pain

Besides, the opioid receptors in the brains tend to be occupied in FM patients - preventing the patients from deriving benefits from more opioids. This suggests that the endogenous opioid system, i.e. the natural opioid system in the bodies of FM (fibromyalgia) patients, is already hyper-active, not hypoactive, and that some FM patients see big improvements in their conditions when they go off opioids. Clauw is not just down on opioids for FM patients, however; he believes they have been widely over-prescribed for conditions they have little effect on.

Cut down on natural opioids: gluten, milk, spinach
Think also about: painkillers!

Consider: neural therapy or low dose naltrexone (check out the special page on the right side of the blog) and not lyrica if you have chronic pain. Antidepressants will modify ultimately the receptors in your gut as well, the place where neurotransmitters for your brain take origin.

Cymbalta’s success in treating pain indicates that some forms of pain in FM are due to central nervous system problems - not a structural injury. Cymbalta’s lack of effectiveness in acute pain , on the other hand, suggests that the two types of pain are very different.

Nieuwe internationale criteria maken scherp onderscheid tussen Myalgische Encephalomyelitis (ME) en Chronisch Vermoeidheids Syndroom (CVS)

Kent de fysiotherapeut de nieuwe Internationale criteria voor het onderscheid tussen Myalgische Encephalomyelitis (ME) en Chronisch vermoeidheids Syndroom (CVS)?

Toen in 1988 de diagnose chronisch vermoeidheidssyndroom voor onderzoeksdoeleinden werd geïntroduceerd ging de gedachte nog uit naar een relatief kleine groep patiënten die niet herstelt van een virusinfectie.

In de loop van de tijd werden deze criteria, Ramsay, 1988 (ME), Holmes et al., 1988 (CVS) Fukuda et al., 1994 (CVS), steeds meer opgerekt en nam ook het aantal verschillende definities voor het ziektebeeld toe. De meest onderscheidende klacht - inspanningsintolerantie - werd niet langer als verplicht diagnostisch criterium gehanteerd. In 1991 introduceerde de Engelse psychiater Michael Sharpe de zogenaamde Oxford-criteria met de focus op onverklaarde psychosomatische vermoeidheidsklachten. Ook psychiatrische patiënten en patiënten met een depressie, burnout of een somatoforme stoornis, werden sindsdien gediagnosticeerd als CVS patiënt. De introductie van soortgelijke Amerikaanse criteria (Reeves et al., 2005) resulteerde in een vertienvoudiging van de oorspronkelijke CVS-populatie (!) zo constateren de Amerikaanse neuropsycholoog Leonard Jason en de psychiatrische-epidemiologe Judith Richman in 2007.

Uit diverse onderzoek van het team van Jason blijkt dat het oprekken van de onderzoekscriteria ten koste gaat van de sensitiviteit en specificiteit (zie bijvoorbeeld). Het werken met een containerdiagnose staat een adequate diagnose en interventie en ook een wetenschappelijke bevestiging van bestaande biomarkers in de weg. Bovendien werkt de triviale naam CVS stigmatisering in de hand. Iedereen is immers weleens moe…

Het lijden van deze patiëntengroep is onevenredig groot in vergelijking tot de omvangrijke groep met sec chronische vermoeidheid. Het verlies aan functionele mogelijkheden bij ME/CVS patiënten is groter dan in het geval van Multiple Sclerose, (eindstadium) nierziekte zo blijkt uit onderzoek van de psychologe Taylor e.a. (2004). De patiënten hebben een verhoogd risico op vroegtijdig overlijden door o.a. hartproblemen (Newton et al., 2011). Desondanks wordt de zorgvraag van de veelal onzichtbare (vaak grotendeels bedlegerige) groep ME-patiënten vaak niet gehoord. Geheel ten onrechte wordt verondersteld dat alle patiënten met het CVS-label kampen met psychosomatiek en/of onverklaarbare ‘subjectieve’ vermoeidheid. Onverklaard blijkt (vaak) wel degelijk verklaarbaar (zie ook: http://www.tcgp.nl/pdf/Pagina_s_van_TcP11-03_web_v3-2.pdf).

Niet alleen patiënten maar ook hulpverleners signaleren dat ‘de vermoeiden’ te gemakkelijk over één kam worden geschoren. Daar komt nog bij dat de meerderheid van de patiënten geen profijt heeft van de in Nederland vigerende behandeling met cognitieve gedragstherapie en graduele oefentherapie. Een behandeling die is afgestemd op patiënten met bewegingsangst en ‘foutieve gedachten’ die de (vermeende) psychosomatische klachten in stand zouden houden. De roep om helder onderscheid te maken tussen de groep patiënten met ME en de (omvangrijke) groep chronisch vermoeiden is de laatste jaren – internationaal - toegenomen.

Ondanks al deze “verwarring over vermoeidheid” (zie de eerdere bijdrage van Speksnijder (http://www.psychfysio.nl/nieuws/2_09_2.html) wordt er wel degelijk wetenschappelijke vooruitgang geboekt. Het onderzoek naar de pathofysiologie van ME/CVS krijgt nieuwe impulsen nu ook toponderzoekers van Harvard University, Columbia University, Stanford University en de Australische Bond University de epidemiologie, het immuunsysteem en de virale pathologie van patiënten onder de loep nemen (zie bijvoorbeeld en bijvoorbeeld).

Een meta-analyse van Cockshell & Mathias bevestigde vorig jaar het bestaan van objectiveerbare neurocognitieve problemen (vertraagde informatieverwerking, problemen met multi-tasking). De Amerikaanse onderzoekers Ocon en anderen laten in september 2011 zien dat de neurocognitieve problemen (ook) voort kunnen komen uit orthostatische intolerantie: de ernstige problemen met de bloeddoorstroming waar veel patiënten mee kampen (zie bijvoorbeeld: http://www.cfids.org/webinar/slides-090110.pdf).

ME/CVS laat sporen achter. Baanbrekend onderzoek van het Amerikaanse echtpaar Light (2011) toont aan dat de verergering van klachten na minimale inspanning middels bloedonderzoek zichtbaar kan worden gemaakt. Australische en Amerikaanse onderzoekers Brenu e.a. (2011) hebben recentelijk (potentiële) immunologische biomarkers in kaart gebracht (zie: http://www.translational-medicine.com/content/9/1/81). De kwalificatie baanbrekend is ook van toepassing op het onderzoek van Schutzer e.a. (2011) met bewijs voor ontstekingen in het zenuwstelsel. Daarmee is overigens nog niet alles gezegd. Onlangs verscheen er een publicatie van de Noorse onderzoekers Fluge e.a. (2011) die het defecte immuunsysteem van de patiënt aanpakken. Zij tonen met een dubbelblind gecontroleerd onderzoek aan dat een chemokuur genezing kan brengen. Vervolgonderzoek is noodzaak.

Dichterbij, in Engeland, is 1,6 miljoen pond onderzoeksbudget voor biomedisch onderzoek beschikbaar gesteld (zie: http://www.mrc.ac.uk/Newspublications/News/MRC008410) aan een onderzoekscollectief onder leiding van Professor Holgate die eerder samen met microbioloog Jonathan Kerr de neuro-immunologische klachten en de aanwezigheid van virussen met behulp van genexpressie zichtbaar wist te maken (zie: http://www.cfs-research.org/publications.htm). Holgate bouwt voort op een langere Britse onderzoekstraditie. In 1999 was het de Britse fysiotherapeute Lorna Paul die samen met neuroloog Wood ‘het vertraagde herstel na inspanning’ objectief wist vast te stellen.

In die context moeten we het initiatief plaatsen van een groep van 25 internationale experts (Carruthers e.a.) om diagnostische criteria te formuleren die helder onderscheid maken tussen de veelal postvirale neuro-immunologische groep ME-patiënten en de patiënten met ‘onverklaarde’ chronische vermoeidheid op basis van een CVS uitsluitingsdiagnose (eerst ‘alle’ bekende andere aandoeningen met gelijkaardige klachten uitsluiten). Clinici in Canada, Nieuw Zeeland en Australië hadden al eerder in 2004, besloten om te werken met de strengere klinische Canadese Consensus Criteria voor ME/CVS. Carruthers e.a. benadrukken in 2011 dat ME (Myalgische Encephalomyelitis) geen onverklaard vermoeidheidsyndroom is en onderschrijven expliciet de door de WHO erkende neurologische classificatie (ICD-10, code G93.3).

De nieuwe strenge Internationale Consensus Criteria (ICC) zijn gepubliceerd in het gerenommeerde tijdschrift Journal of Internal Medicine (zie: http://onlinelibrary.wiley.com/doi/10.1111/j.1365-2796.2011.02428.x/pdf). De literatuurlijst is omvangrijk en maakt o.a. melding van complementactivatie (een ‘allergische’ reactie van het afweersysteem), (verminderde) natural killer cells, ontstekingsreacties, en op scans af te lezen neurologische en neurocognitieve problematiek. In de nieuwe ICC richtlijnen worden de neuro-immunologische problemen, het haperende energietransport en ook de gastro-intestinale en urogenitale abnormaliteiten expliciet benoemd. Onderscheidend en verplichte criterium is de verergering van klachten na - vaak minimale - inspanning, door de auteurs omschreven als Post-Exertional Neuro-immunological Exhaustion. Eind september stonden de nieuwe Internationale Consensus Criteria (ICC) in Ottawa (Canada) op de agenda van een conferentie van de IACFS/ME de grootste en meest toonaangevende wetenschappelijke organisatie voor ME/CVS (zie: http://www.iacfsme.org//).

Behandeling

Deze ontwikkelingen zijn voor patiënten hoopgevend, maar er zijn vooralsnog beperkte mogelijkheden om de ziektelast (iets) draaglijker te maken. Amerikaanse bewegingswetenschappers hebben milde en begrensde oefenprogramma’s ontwikkeld die recht doen aan de inspanningsintolerantie. Thuis oefenen (voor zover mogelijk) kan de problemen dragelijker maken. We komen er in een volgende bijdrage op terug. Daarnaast maken artsen (op basis van lichamelijk-, klinisch- en laboratorium onderzoek) gericht gebruik van o.a. antibiotica, antivirale middelen, probiotica, immuno-modulatoren en remedies ter behandeling van mogelijke oorzakelijke of instandhoudende factoren van de (ME-) klachten. Diverse nieuwe remedies werden besproken tijdens een recente conferentie in Mount Sinai Hospital in New York waar internationale toponderzoekers bijeen waren (zie o.a.: http://blip.tv/mecfscenter/mt-sinai-me-cfs-conference-dr-eric-schadt-5780099)

Vanzelfsprekend is zowel erkenning als kennis en begrip van de ziekte ME bij iedere behandeling een cruciale randvoorwaarde. De patiënt moet er op kunnen vertrouwen dat de behandelaar op de hoogte is van de lichamelijke problematiek en de patiënt serieus neemt. Fysiotherapeuten en ook ergotherapeuten kunnen een begeleidende en ondersteunende rol vervullen. Maatwerk is hierbij het trefwoord.

In de volgende bijdrage, staan we met het oog op de consequenties voor fysiotherapeutische beroepspraktijk, dan ook langer bij stil bij de (objectiveerbare!) inspanningsintolerantie, orthostatische intolerantie en andere symptomen waar ME patiënten mee kampen.

How to monitor chronic Epstein Barr infection?

Voor een wetenschappelijk betrouwbare diagnose en monitoring van de therapie van chronische virale infectie, zoals het Epstein-Barr virus, beveel ik de directe detectie (= antigeen-detectie in het bloed serum) aan.

Antibodies to several antigen complexes may be measured. These antigens are
- the viral capsid antigen,
- the early antigen,
- and the EBV nuclear antigen (EBNA).
- In addition, differentiation of immunoglobulin G and M subclasses to the viral capsid antigen can often be helpful for confirmation.

When the "mono spot" test is negative, the optimal combination of EBV serologic testing consists of the antibody titration of four markers: IgM and IgG to the viral capsid antigen, IgM to the early antigen, and antibody to EBNA.

Abstract

We speculate that the glutathione (GSH) status of human subjects could be an indicator of health and functional age. In this regard, in a study in which, 80 young and 40 elderly healthy individuals were selected as control. We also studied 145 patients with chronic illnesses namely, ischaemic heart disease, diabetes, preeclampsia, cataract, chronic renal failure and leukaemia (age 52 +/- 8.6 years). We observed that all the subjects had high malonadildehyde and low glutathione levels as compared to control. These early observations support the hypothesis that oxidative stress may have an important aetiological rule and antioxidants a potential therapeutic role.

It is very difficult to absorb glutathion via supplements (except IV) but you can raise its level with 8% just by consuming 2 brazil nuts a day. They are very high in natural selenium and this is necessary for the production of glutathion.

"As the integrity of the cellular and subcellular membranes depends heavily on glutathione peroxidase, the antioxidative protective system of glutathione peroxidase itself depends heavily on the presence of selenium."

Unexpected cardiac symptoms with cytomegalovirus CMV infection

Rowe and colleagues [11] described seven nonsyncopal adolescents presenting with fatigue an lightheadedness and demonstrated abnormal responses to upright tilt. They followed this work with a prospective, nonselected, nonblinded study confirming a strong association between neurally mediated hypotention and CFS [12]. An abnormal response to upright tilt was observed in 22 of 23 patients with CFS vs. four of 14 controls (P<.001). Open, nonblinded therapy directed at this abnormal reflex with fludrocortisone, B-adreniergic blocking agents and disopyramide appeared beneficial in this nonblinded trial. The authors concluded that neurally mediated hypotension is a cause of the symptoms of CFS or that it is strongly associated with another important etiologic factor.


We reported in 1993 that repetitively oscillating abnormal T-waves inversions and/or T-wave flats during 24-hour electrocardiogram (ECG) monitoring are present in patients with CFS, ECG abnormalities that are nonspecific but seen less frequently in non-CFs patients (P-<.01) [13]. Abnormal left ventricular myocardial dynamics are present in a cohort of patients with CFS. Decreased and /or flat ejection fractions with stress, , abnormal wall motion at rest and stress, dilatation of the left ventricle, and segmental wall motion abnormalities in CFS patients[14].

We now report a subset of CFS patients with (1) high human cytomegalovirus (HCMV) IgG enzyme-linked immunoassay antibody titers (ELISA), (2) minimal or no serologic evidence of concurrent EBV multiplication, and (3) oscillating ECG abnormalities at Holter monitoring. We performed a pilot study to assess the possible efficacy of ganciclovir, an antiviral nucleoside useful in the treatment of several HCMV infections in immunosuppressed patients.

... Mitral valve prolapse without significant mitral valve insufficiency does not cause abnormal T-wave oscillations at Holter monitoring [19]. As previously, 24-hour continuous ECGs were obtained using a mdified standard lead I and precordial lead V [13]. A patient's 24-h Holter monitor was considered flat, i.e., the T-waves were below the horizontal described by inceptions of p and Q waves in on the the two monitored lead with two or more episodes for at least 25.0 normally conducted QRS complexes. A uniformly flat T-wave were evaluated independent of possible ST segment changes. Biphasic T -wave were considered normal. U waves ( a small, shollow, positive, rounded delflection inscribed immediately after the T-wave) did not interfere with this analysis, Labile T- wave abnormalities at Holter monitoring were present in each CFS patient.


Validation of EI for the severity of the CFS:


Grade 0: Patients are confined to bed by number and severity of symptoms listed below.

Grades 1-2: Any activity leads to overwhelming, incapacitating fatigue. Patients are lightheaded, unable to think clearly, concentrate, or read for any extended period (over 60 minutes). Left-sided chest aches, palpitations, sore throats, and feverishness are frequent. Patients can be out of bed only for intermittent, brief parts of each day.

Grades 3-5: With great effort, patients can be out of bed and perform nonphysical activities for several hours each day. Any exertion markedly worsens fatigue. Patients variably express lightheadedness and inability to think clearly or read normally. Left-sided chest aches, palpitations, sore throats, and feverishness are frequent. Patients cannot perform a 40-hour a week sedentary job or maintain the duties of homemaker (e.g., cooking, cleaning, doing laundry, shopping, driving).

Grades 6-9: Patients can assume normal activities, maintain a 40-hour work week, and with pacing, maintain a household. Overwhelming fatigue is lessened. Rare-to-no lightheadedness, foggy thinking, chest aches, palpitations, feverishness, and sore throats are present. Patients can perform light physical work (or exercise) in moderation without fatigue.

Grade 10: Patients are well, with normal energy level, stamina, and sense of well-being. Exercise in moderation leads to an increased sense of well-being. Lightheadedness, difficulties in concentrating and reading ability, chest aches, palpitations, sore throats, and feverishness are absent.

Treatment:
After placement of a peripherally inserted central or Groshong catheter, ganciclovir (5 mg/kg q 12 h) was given intravenously for 30 days. ... After completion of ganciclovir, patients were encouraged to renew normal activities in gradual increments as tolerated. They were asked to not exercise until six months after completion of ganciclovir.



Results:
At the start of ganciclovir, the severity of fatigue (E1) in all patients was similar (e.g., 3, mean grade, group A vs. 2, mean grade, group B). Six months later, the energy indices diverged. Mean EIs were grade 7 and grade 4 for groups A and B, respectively (table 4). Before therapy with ganciclovir, n patient (group A or group B) could work or function normally (e.g., homemaker).
After treatment with ganciclovir, the 13 patients from group A, but no patient from group B, returned to his or her premorbid activity. A repeat measures analysis of the EI values between cohorts indicated that patients in group A had greater improvement than those in group B (P <.05).
